Eligibility & Screening
Screening, clinical eligibility, and provider approval are always required. Inclusion in a tier does not guarantee appropriateness. Any same-day benefit applies only after enrollment, eligibility, and terms acceptance.

Rollover & Pauses
Eligible unused included sessions may roll forward up to 30 days, subject to written terms, availability, and good standing. After the initial three months, one requested pause of up to 60 days per membership year may be approved in writing, subject to account good standing and the controlling agreement.

Locations & Use
Eligible services may be used at either the Rancho Palos Verdes or West Palm Beach clinic, subject to service and location availability. Membership is non-transferable except for the explicit guest benefits stated in the tier.

Pricing & Discounts
Member discounts apply only to eligible services and generally cannot be combined with other offers. Tier changes take effect on a future billing cycle, subject to approval and terms. Annual prepay, if offered: prepay 11 months and receive 12, subject to a written agreement.

Rate Protection
Continuous-member rate protection applies only while continuously active and in good standing. It ends after cancellation, lapse, or termination and does not override taxes, third-party charges, or material scope changes.
