No Doctor–Patient Relationship

Browsing this website, submitting a consultation request, exchanging messages with our concierge team, or speaking with staff by phone does not, by itself, create a doctor–patient relationship. A clinician–patient relationship is established only after an in-person or telehealth evaluation with a licensed provider at one of our clinics and completion of the appropriate intake and consent forms.

Our services are wellness-oriented and physician-supervised where required by law. Nothing on this site should be interpreted as a promise of a specific medical outcome. Any decision to proceed with a therapy is made together with your provider based on your individual history, screening, and goals.

Third-Party Content and Sources

Where we cite peer-reviewed literature or government sources, those citations describe general research findings only. They do not prove that any particular therapy will produce a particular result for you. Links to third-party websites are provided for convenience; we do not control and do not endorse the content on those sites.

Appointment, Deposit, Cancellation & Refund Policy

This section will be published once the following 14 business decisions are confirmed by the owner. Until then, deposits and cancellations continue to be handled by phone with our concierge team — please call the clinic directly.

  1. Legal entity that appears on the cardholder’s statement (per clinic).
  2. Deposit amount (flat $100 across all services, or variable).
  3. Whether the deposit is applied to the treatment total or is a separate booking fee.
  4. Cancellation window for a full refund (e.g. >48h) and partial-refund tiers.
  5. No-show rule and any grace period.
  6. Reschedule rules — how many reschedules per deposit, transfer window, transfer to another person or service.
  7. Late-arrival threshold (minutes) after which the visit is treated as a no-show.
  8. Clinic-initiated cancellation — refund and rebooking priority.
  9. Refund handling when a medical contraindication is identified at screening.
  10. Whether minors are treated and, if so, guardian consent and payment rules.
  11. Refund method (original card only) and business-day SLA.
  12. Payment surface of record (Boulevard, Square, Stripe, phone-only, other).
  13. Confirmation channel of record for the deposit receipt (payment processor, Info@ebo2therapy.com, or both).
  14. Effective date for policy v1 (recommended: 7 days after publication).

Once these are confirmed, the policy will be published here with a matching required consent checkbox on the consultation request form, and the version, timestamp, IP address, and user agent of each acknowledgement will be stored for our records.

Deposit Authorization Summary

When a deposit is collected by our concierge team (by phone or in clinic), you are authorizing a one-time charge to the payment method you provide for the amount quoted at the time of scheduling. Full authorization language, cardholder receipt, and any card-on-file terms are provided at the point of payment and, where applicable, on the signed intake form. The complete deposit, cancellation, and refund terms will appear in the section above once finalized.
